Complete Trails A, Trails B, and NSI using one patient demographic entry. NSI scoring is calculated automatically after all items are answered. After all sections are completed, select Submit Results to generate the branded PDF and submit it to Intake.
Trail Making Part A
Select numbers 1 through 25 in order as quickly and accurately as possible. The correct next item will only highlight after 2 errors.
Trail Making Part B
Select alternating numbers and letters in order: 1 → A → 2 → B → 3 → C, continuing through 13. The correct next item will only highlight after 2 errors.
NSI Neurobehavioral Symptoms
Scale: 0=None, 1=Mild, 2=Moderate, 3=Severe, 4=Very Severe.
